(Video) Manchester City substitutes combine for opening goal against Chelsea

Manchester City are taking on Chelsea this evening and they have taken the lead midway through the second half.

After a goalless first half, Pep Guardiola rolled the dice on the hour mark and brought on Jack Grealish and Riyad Mahrez to try and impact the game.

And that is exactly what they did as City created a swift, flowing move which ended with Grealish rolling the ball across the six-yard box for the Algerian to tap home.
